nurses at Community Health
and Education Services
(CHES) are here to
administer a customized care
plan.
The service is tailored to fit the
needs of Medicaid enrollees
receiving support through the
Developmental Disability Agency,
as well as under the Community
First Choice program.
CHES' nursing support is
available to patients who want
self-directed services (SDS).
The team reviews health risk screening tools,
develops healthcare protocols, offers community
health-related resources, and establishes lines of
communication with care providers.
CHES also appoints registered nurses (RNs) who consult you and your
Primary Care Provider to deliver an individualized treatment plan
attuned to your needs.
These patient plans are regularly reviewed by CHES - the team
monitors the quality of the services provided to deliver optimal
care.
