nursing support services for clients with Cerebral Palsy, spina bifida, and
those on the autism spectrum.

The team works with the
client's Primary Care Provider
to design a customized plan of
care that supports daily
activities, events, and all other
childhood moments.
The group provides
nursing consultations and
nursing case
management and
delegation solutions, all
available to Self-Directed
Services (SDS)
members.
CHES uses the
Health Risk
Screening Tool
(HRST) to identify or
review your child's
health care level.
The group also drafts healthcare protocols, establishes effective mechanisms
for communication with healthcare providers, and finds health-related
resources in your community.
Its team makes nursing and self-administration assessments and oversees the
training and work of the staff authorized to implement the tasks detailed in the
client's care plan.
